Program Statement


Willow Winds Living, LLC, is a Licensed Adult Family Home Community, under Wisconsin Administrative Code DHS 88, to serve up to four (4) men and/or women 18 years or older, with Advanced Age, Developmentally Disabled, Emotionally Disturbed/ Mental Illness, Traumatic Brain Injury, and Terminally Ill, per house hold.  Located within Jefferson County, off Hwy 12, on Twinkling Star Road, Whitewater, WI, 53190.

The homes Willow Winds Living, LLC are secluded ranch style duplex homes, located just minutes from the town of Whitewater.  Each home offers 3 private bedrooms, 1 private suite, at least one shared bathroom, a large kitchen with breakfast bar, spacious family room with fireplace, as well as laundry facilities on-site.  A sun filled deck is located through each patio door of the family room and over looks the large, open yard.  We are a pet friendly community and have regular visits from our four legged friends. The property features (2) ever growing community gardens, (2) campfire pits, tree swing and hammocks, as well as nature trails through the woods for relaxing walks which allows for a unique country living atmosphere.  In 2017, we are welcoming our first livestock to our homestead community, chickens and ducks! This will allow our residents to not only enjoy a farm atmosphere. But also allow them to gain valuable life skills knowing where their food has originated and assisting with the process of bringing the items from farm to table.

The homes at this time is not equipped for residents in wheelchairs or for those who walk regularly with the use of crutches or a cane.

Program Goals:

The overall goal of Willow Winds Living, LLC, is to maximize each resident’s cognitive, affective, physical health and psychomotor skills in order to help them obtain their highest possible level of functioning and independence.  This will be accomplished through our service principals provided by are compassionate and outgoing staff:    

·         Celebrate the individuality of each resident

·         Respect the right to privacy and independence

·         Preserve the dignity of each resident in our care

·         Involve family and loved ones in care planning

We provide attitudes of hopefulness, a structured, yet warm environment, individual treatment plans with realistic goals, and the opportunity for group living and social interaction.  Essential to our program is an emphasis on tasks of daily living along with community integration, such as religious activities, cultural events, and social interaction through community events and activities.  Each resident will have a goal oriented Individual Service Plan that will be discussed and reviewed with case managers, guardians, families, and others involved in their lives.

Specific Program Goals Include:

·         Assisting and training residents in personal care activities such as dressing, grooming, and bathing.

·         Assisting and training residents in daily living skills such as meal planning and preparation, laundry, and household chores.

·         Assisting residents in appropriate socialization, including productive and independent leisure time, one-on-one interaction, group activities, as well as community work or day programs.

·         Assisting residents in understanding and expressing their emotions and feelings, focusing on building good self-esteem and sense of self-worth.

·         Encouraging and helping residents in maintaining involvement with family members and other significant people in their lives.

·         Accurately assessing and responding to the unique and changing needs of each resident.

Staffing Level and Training:

Willow Winds Living, LLC will have at least one staff on duty at ALL times to ensure our program goals for each resident are met.  The ratio of staff to residents shall be adequate to meet the needs of the residents as defined in their assessments and current Individual Service Plans.

Required Staff Training:

·         First Aid & Choking

·         Fire Safety

·         Medication Monitoring

·         Standard Precautions

Our staff consists of trained Resident Assistants and/or Certified Nursing Assistants with CBRF certifications.  As well as students and interns from the University of Wisconsin - Whitewater Special Education/Social Work Programs, who enter our Resident Assistant or Management Training Programs.  Staff are trained in First Aid, Medication Monitoring, Fire Safety and Standard Precautions and additional 8 hours of training is offered throughout the year for each staff member within the client groups served.


Willow Winds Living, LLC will provide transportation to appointments, outings, etc. and although we primarily use company vehicles, we do have the right to transfer transportation duties to service providers with a valid driver’s license and proof of liability insurance.

Safety Issues:

Willow Winds Living, LLC, has emergency plans in case of fire or severe weather, necessary alarm systems, community safety as well as prevention of sexuality and abuse issues.

Health Monitoring Services:

Health monitoring services are provided by your own physicians and nurses, or those within the community, who ensure that each resident receives an initial health screening assessment and at minimum, annual examinations.

Medication Monitoring:

Medications are dispensed by staff.  As well as monitored in a locked area by trained staff.  Medication administration by authorized staff will occur only when ordered by a physician and as the order states.

Leisure Time Services:

Leisure time services are designed to maximize meaningful and productive activities in the community as well as at home.  Residents have the opportunity to structure their leisure time individually and through group consensus.  Activities may vary from a variety of day programming options in the community, to at home activities such as music, cooking, crafting, exercise and visiting the library, local shopping malls, restaurants, and nature hikes.  As well as, picnics in several area parks, sporting events, special community events.  Special events and tours are scheduled throughout the year.
